[PATCH] USB: storage: Add unusual_dev SINGLE_LUN entries

This patch adds entries for several USB floppies that need
the US_FL_SINGLE_LUN flag. These were reported by
Sebastian Kapfer <sebastian_kapfer@gmx.net> and Olaf Hering
<olh@suse.de>, with rediffing and cleaning from me.
