[PATCH] pcmcia: id_table for pcmciamtd.c

Add pcmcia_device_id table to pcmciamtd.  The binding of anonymus cards (i.e.
those who do neither report MANFID, CARDID, FUNCID nor product strings) is
protected by a new config option.
